ELIGIBILITY REQUIREMENTS

In order to be eligible to try out for an athletic team, a student must:

1. 1.Pass 4 subjects in the previous grading term.
   

2. 2.Have a signed copy of the BMR physical eligibility form.  The copy must be signed by the school nurse.
   

3. 3.Have a completed “Athletic Permission/Emergency Form.”
   

4. 4.Have a signed “Handbook Acknowledgement Form”

NOTE:  *The above conditions must be met in order for an athlete to participate in the tryout process.

**All forms can be found outside Mr. Johnson’s office in the lower B wing or can be downloaded.
